Fill out the IPOPHL-USPTO PPH Request Form
Attach the following:
- Copies of all office actions, which were sent for the corresponding application by the Office of Earlier Examination (OEE)
- An information disclosure statement listing the documents cited in the PH office action
- Copies of all references cited by the OEE examiner (Note that any document copies submitted in the U.S. application prior to the Global/IP5 PPH request need not be resubmitted.)
- Claim Correspondence Table
Submit the form and attachments to the Receiving Office. No filing fee required.
